How to activate your WVA Buying Group Account

1. Fill out WVA Customer Credit Application.
2. Sign and date the application.
3. Return the application via fax to (262) 763-0114.

Upon approval of your application WVA will contact you and provide your new WVA Buying Group Account Number.

This account will also be used for all of your contact lens purchases.

Already ordering contacts through WVA?

If you are already ordering contacts through WVA you don’t need to do any additional work.  Just tell the vendor the next time you place an order that you would like to be billed through the WVA Buying Group.  The vendor will call us to verify your membership and all future purchases will be billed through us.
